Deadline for application is October 15, 2008 – applications must be postmarked no later than October 12, 2008 to be considered. Please be sure information is complete, correct and legible – program rosters will be taken directly from information submitted. Telephone, fax and e-mail information is especially crucial for accurate and timely communication. If you decide to fax or e-mail the application or (roster information out of state teams), please mail the originals along with your check. Teams are guaranteed a minimum of 4 pool play games – the top two teams will compete in the Championship game. Your entry check will be returned to you if you are not accepted into the tournament.
